How to prevent hiking blisters is a common question. Hiking blisters are every hiker's nemesis, those small, fluid-filled sacs that can turn an exhilarating adventure into a painful slog.

Whether you're on a multi-day trek or a short day hike, blisters can strike anyone, even the most experienced hikers. Knowing how to prevent and treat hiking blisters is essential to keep you on your feet and enjoying the walk

What Are Hiking Blisters and How Do They Form?

Hiking blisters form as a result of friction or pressure on the skin. The friction causes the outer layer of skin (epidermis) to separate from the inner layers, creating a small pocket that fills with fluid. This fluid, known as serum (a type of plasma), is your body's way of protecting and healing the damaged tissue. 

Blisters can also form due to burns, allergies, skin conditions, or even insect bites, but in the hiking world, friction is the most common culprit. When you're hiking, the repetitive motion of your feet against your boots, combined with pressure points and moisture, creates the perfect storm for hiking blisters to develop.

The 4 Stages of a Blister

Hiking blisters progress through four clearly defined stages, and understanding these can help in their management. Here they are:

  1. The Hot Spot Stage: A hot spot is your body's signal and a warning sign that the skin is under stress. A red, tender spot may form due to the beginning of separation between skin layers.

    The hot spot stage is important for blister prevention because taking immediate action, such as applying a protective bandage or adjusting footwear, can stop the progression from hot spot to blister. It's a reactive phase where recognising the early signs of friction can save you from a nasty blister.
  2. Blister Formation with an Intact Roof: Once a hot spot goes unaddressed, that irritation can escalate from hot spot into full-blown hiking blister. And this can take a matter of minutes.

    At this stage, the separation between the epidermis and the lower skin layers has advanced, and fluid (serum) fills the space to cushion and protect your deeper tissues.

    The “roof” of the hiking blister is the top layer of skin that holds the serum, creating a natural bandage. This stage can be painful, but the intact roof is vital for preventing infection and further damage. The fluid, as I mentioned before, resembles plasma, and it provides nutrients and signals the body to start the healing process.
  3. The Torn Roof Stage: This phase is more precarious as the integrity of the blister ‘roof' becomes compromised. The roof typically tears due to continued friction or pressure – perhaps from further activity, tight footwear, or the blister has been “pricked” or lanced.

    An opening in the hiking blister roof can be a gateway for bacteria, increasing the risk of infection. At this point, efforts should shift to protecting the area to keeping it clean and allowing healing to continue. The focus being on preventing bacteria from entering the wound.
  4. The De-roofed Blister Stage: This is when the blister roof comes away. Typically, this occurs when an incorrect dressing (or no dressing) is applied. When the wrong dressing is used, it can pull off the roof when removed.

    With the full removal of the blister roof, the sensitive lower skin layers are exposed. This is the most vulnerable stage for infection because the natural protective barrier is gone.

    The de-roofed blister is painful, and the new skin underneath is delicate and raw. Care for a de-roofed blister involves meticulous hygiene and protecting the area with an antiseptic ointment and a sterile dressing. You need to minimise the pressure and friction to allow the area to regenerate skin effectively.

So Why Do Hiking Blisters Form?

Friction and Shear Distortion

Most people believe blisters occur due to a rubbing friction. However that is not necessarily the case. It's friction known as Shear Distortion that causes blisters.

Shear distortion happens when your skin moves in one direction while the underlying structures, like bones, move in another, often due to friction or pressure. This mismatch in movement can easily lead to skin irritation (hot spots), and subsequently damage and blisters.

Tight spots in your boots or wrinkles in your socks can create pressure points that lead to shear distortion. When sweat builds up, skin becomes tacky and less able to slide smoothly, increasing friction and making it harder for your foot, sock, and boot to move naturally against each other. Any movement that does occur happens later and with more force, and is the perfect setup for a blister.

How to Prevent Hiking Blisters Before they Start.

Preventing hiking blisters is all about awareness, preparation, and taking proactive steps to minimise blisters when hiking. Here are some tried-and-true methods to keep your feet blister-free:

Choose the Right Footwear

Proper Fit: Make sure your footwear fits snugly but not too tight, allowing enough room for your toes to wiggle without sliding around. When shopping for new booking footwear, try them on in the afternoon when your feet are slightly swollen, along with hiking socks, to ensure the best fit.

There is a movement towards Trail Runners with wide toe boxes, allowing your toes movement within the shoe, while still being supportive. Topo or Altra Trail Runners are both highly regarded with their roomy toe box.

Break Them In: Never take brand-new boots or shoes on a long hike. Break them in by wearing them on short walks, or during day hikes. This will help your footwear conform to the shape of your feet and allow you to note any potential hot spots for blisters before you set out on a long hike.

Wear the Right Socks

Avoid Cotton: Cotton socks retain moisture, which can soften your skin and increase friction. Opt for moisture-wicking materials like performance synthetic blends or merino wool socks such as Darn Tough or Smartwool, which help keep your feet dry and comfortable.

Liner Socks: Thin liner socks such as Injinji toe socks, are worn under your regular hiking socks. These toe socks provide an extra layer of protection by reducing friction and wicking away moisture.

Change Socks Regularly: On longer hikes, carry an extra pair of socks and change them when your feet start to feel damp. Fresh socks can make a world of difference in preventing blisters as the moisture causes the skin to become clammy or tacky or sticky, which adds to the probability of shear distortion (friction).

Keep Your Feet Dry

Foot Powders: Use foot powders such as Gold Bond Powder or Anti-chaffing Powder or anti-perspirants to keep your feet dry and reduce moisture buildup. These products help absorb sweat and are particularly good to use in hot, humid conditions.

Air Out Your Feet: Take breaks during your hike to remove your boots and socks, allowing your feet to air out. This not only helps keep them dry but also gives you a chance to inspect for any developing hot spots for blisters.

Manage Hot Spots

The moment you feel a hot spot (a precursor to a blister), stop and address it. Hot spots are areas where the skin is irritated but hasn't yet formed a blister. Studies have shown you have less than 15 minutes before damage between the epidermis (top layer of the skin) and the underlying skin tissue develop. Timing is pretty much important.

Protect Hot Spots by applying blister prevention tape (Rock Tape is excellent), moleskin, or specialised blister bandages such as Engo Blister Patches to hot spots as soon as you notice them. These products provide a protective layer that reduces friction and prevents further irritation.

Use Blister Prevention Products

Blister Kits: Carry a blister prevention kit that includes items like moleskin, blister tape, gel pads, and antiseptic wipes. A Pre-made Blister Kit includes everything you need for prevention and treatment, or you can make your own up.

Blister Wool: Blister Wool or Hiker's Wool works by providing a protective padding that helps reduce friction and pressure on hot spots and blisters.

By placing a small amount of hiker's wool over a blister or a potential blister area, it can help alleviate discomfort and prevent further rubbing, allowing the skin to heal more effectively. The natural lanolin in the wool also helps keep the skin moisturised and promotes healing.

How to Treat Hiking Blisters

Blister Prevention suggests the 5-Step Method, which includes the following steps:

The 5 Step System

  1. Identify Your Blister Stage
  2. Apply An Antiseptic
  3. Choose The Right Dressing
  4. Change Your Dressings At The Right Time
  5. Monitor Regularly For Infection

Step 1: Identify your Blister Stage

Your blister will be at one of the four stages as mentioned above.

  • A hot spot is the pre-blister stage. It is your body's warning sign that you need to act immediately. A hot spot will feel warm to the touch and slightly painful.
  • The blister will be intact with the roof of the blister (that's the top layer of skin) totally covering the liquid inside and no weeping of the blister. Any weeping of liquid will suggest a torn roof.
  • The roof is torn, but still covering the sensitive layer of skin underneath. The liquid has seeped out of the wound.
  • The blister is de-roofed – which means the top layer of skin has been removed.

Step 2: Apply An Antiseptic

If your blister is torn or de-roofed, to prevent any infection, it is crucial to apply an antiseptic. Reason being is that the de-roofed blister stage exposes sensitive lower skin layers, making it vulnerable to infection. An infected blister can quickly escalate to septicemia, a serious blood infection, if left untreated.

My preferred antiseptic is Betadine, which contains povidone-iodine that kills bacteria, viruses, and fungi by releasing active iodine that penetrates microbial cell walls and disrupts their function.

It does leaves a brown stain on the skin, which is actually a useful visual indicator. When the colour fades, it usually means the iodine has been depleted, and the area may need to be cleaned and re-treated.

If you don't have Betadine, antiseptic wipes will do just fine. Just be aware they may sting a bit.

If your blister is intact, there is no need to apply antiseptic as the intact roof is acting as a barrier to keep germs out. However, if your blister is showing signs of infection, it's time to use an antibiotic cream and seek medical advice.

Step 3: Choose the Right Dressing

Hot Spot: Stop immediately and apply a preventative measure to the area. Tape, lubricant, fresh socks. This is the perfect opportunity to use items in your Blister Kit

Intact Blister: Apply an Island Dressing and monitor for changes. Ensure the pad is large enough to cover the entire blister, otherwise the adhesive will tear the roof of the blister.

What is an Island Dressing?
An island dressing consists of a non-adherent pad surrounded by an adhesive border. The non-adherent pad, is centered on the wound to protect it while the adhesive border secures the dressing in place. This design helps prevent the dressing from sticking to the wound, making it easier to remove and reducing the risk of further damage when changing the dressing.

Torn Blister: After applying antiseptic, apply an Island Dressing – again ensuring the adhesive does not touch the blister roof – even though it is torn. DO NOT apply a Hydrocolloid blister plaster, such as Compeed (see below).

De-Roofed Blister: Apply a Hydrocolloid blister plaster such as Compeed.

Hydrocolloid blister plasters are specifically designed for treating de-roofed blisters. These adhesive, waterproof dressings maintain full contact with the wound and can remain in place for several days. By interacting with the weeping blister base, the hydrocolloid particles in the plaster create an ideal healing environment for the blister.

As hydrocolloid blister plasters are fully adhesive, using them on torn or intact blisters will result in the roof being removed when the dressing is changed.

Step 4: Change Your Dressings At The Right Time

Island Dressings: Change dressings when you can see fluids such as blister fluid, blood or water coming through the gauze pad. Once this happens, it's vital to change as the moisture from the fluid creates a path for bacteria to enter the wound.

Hydrocolloid Bandages: Hydrocolloid bandages turn white as they soak up the liquid from wounds, causing the dressing material to swell and change colour. This colour change shows that the bandage is working to manage the wound fluid and create a moist environment that helps with healing.

Once the white “bubble” has reached the edge of the bandage it is time to change the Hydrocolloid bandages Hydrocolloid bandages can last for up to a week – however monitoring the bandage and wound is really important. Once the white bubble reaches the edge of the bandage the seal is broken and it is an entry point for germs.

Step 5: Monitor Regularly For Infection

Infection: If a blister becomes red, swollen, painful, or filled with pus, it is infected. Red streaks spreading from the blister toward the nearest lymph node are also a sign of infection and need urgent medical attention.

Red streaks spreading from a wound towards the nearest lymph node can be a sign of lymphangitis, which is the inflammation of the lymphatic channels. If left untreated, lymphangitis can progress to a more serious condition called sepsis or septicemia, where the infection spreads throughout the bloodstream and can be life-threatening. Therefore, prompt medical attention is crucial if red streaks are present, as it may indicate a potential risk of developing sepsis.

Large or Severe Blisters: Blisters that cover a large area, are extremely painful, or show no signs of healing after a few days should be evaluated by a healthcare professional.

Underlying Health Conditions: If you have diabetes or other conditions that affect circulation or wound healing, take extra care with blisters. Seek medical advice if you develop a blister to avoid complications.
